When calling to speak to someone in Billing about how my case was coded for submission to my dental insurance, the clerk would only give me her name as "D." Not "Dee," as in Sandra, but just D. No last name. Wouldn't give me her last name. Informed me that was how she was known only in the office for these purposes.
