I made an appointment with this doctor for weight loss. When I arrived for my appointment, the receptionist had me fill out an intake form. On this intake form, it asks me if I'm a member of a government organization or if I'm a reporter for the media. I thought this was strange but overlooked it and of course answered "No".

The form then asks me for references and relatives that don't live with me along with the names of my immediate family, including my children. I can understand an emergency contact number but fail to understand why this is necessary.

Most importantly, the form asks for your social security number. The references and the social security number would be understandable if I was going to be billed after treatment or if insurance was being used, but insurance is not accepted by his business and everything must be paid for upfront.

At this point I'm just a hair past curious but no real bells are going off. I decided to ask the receptionist, politely I might add, why my social security number is needed, assuming there would be a good reason. However, I was told that my social security number is only needed for filing purposes. She explained that this is how they reference my "account" and pull up my information within their system. I asked her why it is that they can't use my name, phone number or even my driver's license number and she explained that this is simply how the system works. Given that I wasn't satisfied by her answer, she offered for me to provide my DL number instead, to which I was grateful and satisfied. The conversation ended without tension and I proceeded to complete the form provided without providing he vital information Dr. Meeks asked for.

While I'm completing my intake form, Dr. Meeks tells the receptionist that when someone starts to question the intake form to tell them to leave. Not kidding. He then tells the receptionist, right in front of me where I can clearly hear him, to tell me to come back to his office. Thinking that things are fine and that we're about to begin my consultation, I address him as sir and ask him how he's doing. He ignores my question by replying, "yeah, you know, I usually wouldn't take people like you..." in the rudest tone I've heard from any professional. Before he could explain how he planned to extend mercy to me, I told him that was fine and walked out of his office. He was livid.

I'm not here to say that Dr. Meeks has any ill intentions or that he aims to steal someone's identity or anything, but they should be understanding, especially in this day and age, why someone would inquire about the need for vital information when credit is not needed or even optional. He was extremely rude, condescending and creepy to say the least. The bottom line is, social security numbers and the names of my children are not necessary for prepaid services. They ought to come up with a good excuse if nothing else.
