Dr. Jacob Kozacki is a board-certified family medicine physician and native of Florence, SC. He graduated from Clemson University and attended medical school in Spartanburg, SC, at Edward Via College of Osteopathic Medicine. He subsequently returned to his hometown and completed his family medicine residency training at Mcleod Regional Medical Center.
