Formerly Asta Care of Pontiac, purchased in October 2014 by Certified Health Management, renamed Pontiac HealthCare and Rehab. Under new ownership and new management Pontiac HealthCare and Rehab recently underwent their change in ownership survey, which is deficiency free. Pontiac HealthCare and Rehab is focused on improving not only the building environment and Rehabilitation services, but making this home a wonderful place for the area seniors needing skilled care, short term rehab, or long term care. Our philosophy is that we work in our residents home and we want them involved in the changes going forward, so as to enhance their quality of living. Please feel free to stop in and visit, take a tour and ask about the many changes we are making to insure our residents are enjoying their time here.
