I plan on telling everyone I know about how terrible this nursing home is. I wouldn't recommend this place to my worst enemy. My mom was there for therapy for compression fractures of the spine she was there 12 days and was never seen by a doctor On the 12th day when she needed ti be sent to the hospital it took a long time to contact their dr, to get permission to send her and she died enroute to the hospital then it has been a fight for 3 months to get a death certicate and they have been very mean about it.

My Mother was sent to Primacy Healthcare & Rehab Center for physical therapy rehabilitation after fully recovering from pneumonia in the hpspital. Although she received very helpful physical therapy and occupational therapy services almost daily while at the facility, and we were pleased with that part of her care, in general, we noticed immediately that the facility was not very cautious in and around her room area or the common areas with cleanliness and the spreading of infections and disease. The use of gloves, hand washing, and sanitizer gel was almost never seen, and in the end, 12 days after the prescription for antibiotics that she came into the program on had ended, she came down with Clostridium difficile(C. diff.). She was not well attended at Primacy when she came down with fever and other signs of this infection, and after three days, we felt we absolutely had to remove her from Primacy, and took her back to the hospital for treatment of this very serious infection...where she remains fighting this infection after leaving Primacy 22 days ago.
