Not trustworthy. Lies & misrepresents info to keep work comp insurance happy. Knowingly submitted eval with errors & false statements w/o correction after initial mtg then let me go as a patient when I tried to address the issue. Combative when confronted. Passive aggressive. Takes advantage of trust of patient & did not provide full informed consent either. Held on to informed form having me sign it under distress while holding on to it without ability to read it.
