This unique program is designed to increase positive outcomes for those individuals living with diabetes. It places focus on not only eliminating and/or reducing the chronic effects of the illness, but reduce the escalating medical cost to the patient and managed care companies.
P : Provide high quality diabetes testing supplies delivered directly to the patients home which has been proven to increase compliance rates:
Blood Glucose Meters
Testing Strips
Lancets
Lancing Device
(All major brands including the Prodigy Talking Meter)
E: Educate the patient on how to live with diabetes by providing monthly lessons on meal planning, nutrition, exercise, blood testing, physician communication (eyes, foot care, nerve damage, wound care, etc.) All education is provided by Registered Dieticians and Certified Diabetic Educators.
R: Report the risk level results based on monthly monitoring of data collected from the patient and physician. Reports demonstrate compliance level trends (i.e. elevated A1C, weight gain, emergency room visits, damaging diet, etc.).
T: Team with the health care providers and managed care plans to review patients with High Level Risk Assessments to proactively develop a plan of action to increase outcomes and reduce the high rising medical cost.
We also provide Diabetes Testing Supplies to individuals who DON'T HAVE INSURANCE COVERAGE.
