Care was good, but gradually deteriorated. Despite my financial account being in good standing, during a phone call her nurse simply dropped me as a patient with no plans for follow-up as I was transitioning to a new medicine. Abrupt discontinuation of care with no offer of transition- without even the offer to assist in transfer of care to another provider- meets the standard of malpractice. Many hours later, this same nurse called back to do "damage" control, but, with their level of dedication to patient care already exposed, I had to then take it upon myself to terminate my care there. Now, I am meeting difficultly in even getting medical records sent to me. I sent in the State of Minnesota release of information form, but received a letter indicating that I had to use their form despite the past transfer of records with the State's form. And, of course, no apologies from anyone where Dr. Lombardo now serves as the President of this organization.
