My husband died alone in this horrible place. It is disgusting. The staff needs to be fired and this place needs to be FINALLY SHUT DOWN. There is no way they are getting licensed legitimately. The rooms are smelly and old. The floors and bathrooms are dirty. They don't give a care about the patients. Their website is nothing but lies!! This is a place you leave someone to die. Please do not entertain this dump. I wish someone had warned me of this place, my husband would still be here.

Don't do it, step away. Never seen such a place, like this. in my whole life. Better off recovering at home, with home health and other therapy. The place is filthy, no voicemail to leave messages, poorly trained staff/ risk of over medication. Puts patients at Risk of any form of infection or virus

PLEASE do not take your loved ones to this facility; it is horrible. Over half of the staff has an attitude and rarely do any work. Half of the time, you can't even find them. I can count on one hand the number of employees that do their job over there. A resident can go all day without the aide ever coming in the room but will appear at the end of the shift to maybe empty the foley bag. Some aides and the nurse only make one round during a 12-hour shift. Oh, and don't worry about going to the administration or the DON because they are aware of what's happening; they just don't care.

DO NOT SEND YOUR LOVED ONES HERE. My parent was in here for covid recovery. They do not care for the patients. They never answer the phone or call back. The respiratory staff is incompetent. What should have been rehab to home turned into rehab to grave. They ignored my moms infection until she got septic and ended up passing away. I had to beg for them to bathe her. She had knots in her hair from not being brushed. When I tried to get her out into hospice because she was DYING obviously they wouldn't send the proper paperwork to the company. they made sure that they procrastinated until her 90 days of coverage from medicare had ended. When the money ran out they didnt want her anymore and basically killed her. I didnt want to give it a rating at all but it made me. - 1 million stars

Our family decided that we needed to put my Mom in the nursing home. Collierville Health and Rehab was the closest for our family to be able to visit. I read reviews and was skeptical about that home. I decided to go in person for a visit and see for myself. On the day I went, I met with Lisa, whom I believe was the director (not real sure) She was the sweetest person and so helpful. She walked me through the facility and answered all my questions. After going there in person and meeting the care takers and seeing the place was being remodeled, I called my brother to let him know that I was very pleased with what I had discovered. Lisa was so helpful with getting my mom admitted and helping with the paperwork. (Thank you Lisa) I'll be looking for you each time I visit my Mom. I'm sure Mom is going to be very happy there. I will write another review after she has been there for a while to share my thoughts. Again Thank you Lisa and staff for being so sweet and helpful !!!! Looking forward to hearing my Mom's opinion in a few weeks

terrible culture and atmosphere of non-caring individuals. facility is ALWAYS poorly staffed and the staff present are under the impression that they do not have to work. I don't understand how you can bear to leave someone, in a helpless state, in a poor state of care. The state needs to come in and rectify the many staffing issues. Not to mention in the 4 months I have visited, I have only seen cleaning staff twice. And they had an asbestos breakout last month that I never saw any licensed official remedy. They literally painted over it!
