If I could rate them in the negatives (i.e. -100) I would. Our mother was transferred to this place from a hospital. She suffered a stroke and required rehabilitative services. This facility wreaked of urine. Patients yelling from their rooms needing help. Rooms were absolutely disgusting. My mother's shared room had a sink that was clogged up and had what appeared to be throw up in it. The bathroom literally had poop on the toilet and the bed sheets had blood stains on them. My mother required constant watch - which they could not extend to her. She is a feeder, which means someone needs to help feed her. She is a diabetic and the food that they would give her would made her blood sugars rise really high in the 400's and to the nurses, that reading was good. What nurse is trained to read such high results to be a good reading? They were not willing to change the diet and would not allow her visitors to bring her food. Speech therapy at the hospital did a swallow test on my mom and wrote up that my mother would need to have her food finely chopped. This place served my mom whole foods making it challenging for her to chew / swallow. My mother required physical therapy, occupational therapy, speech therapy and pelvic floor therapy. We were told initially that she would be in good hands and that they have over 100 staff members including physicians that would be visiting with my mom. My mother has been there a week now and not ONE doctor has come to visit with her. Physical therapy came to visit her one time and that was to do an evaluation on her. The nurses and PCTs/CNAs were very bothered with having to attend to my mother. They would move the call button over her head making it nearly impossible for her to reach it. My mother is incontinent. We kept my mom on video camera via a tablet to keep an eye on her when we were not there. My mother would ask us to have the nurse come in to change her as she dirtied her underpants. In one instance, it took them over 4 hours to come into the room and that's that we kept calling and calling until someone came into the room. My mother has formed a rash in her private area and came in with a wound in her rear end - which got worse being at this center. They refused to treat the wound with the PRESCRIBED ointment. One day, I went in only to find that she was plastered with a bowel movement and completely wet with urine. Fecal matter on her wound. How is that sanitary?!? She ended up with a rash in her private area. My mother's room was also across the hall from 2 rooms where there were Covid patients that should have been in isolation. Their doors were kept wide open. It wasn't until we informed the director of the place that they took action and closed their doors. Director stated that she was unaware of this, but if you are running this place, how do you not know what is going on in YOUR center?!?! There was also a CNA that you could hear coughing down the hallway that would come to my mother's room without a mask. We asked repeatedly that whenever attending to our mother that they wear a mask. According to the attending nurse, the CNA is to wear a mask at all times. We had to constantly remind the nurse to please give our mom her medicines. It should not take a patient to remind the nurses to give them their medicine. They have the med list from the hospital, they should know when and how to give it to the patients. My mom's neuro encouraged us to stimulate my mom's brain by queuing her up for responses and to interact with her as much as possible. Under the recommendations of that neuro, we setup a tablet for her to where she can video chat with all of her family members. The staff members would come into the room and close the tablet on us / her and we would constantly have to call up to the center and reiterate the neuro's recommendation and ask them to please reconnect us to her. Those asks never went easy. Prior to my mom being admitted to this place, we contacted the admin and was told that she can have the tablet in her room. So why was it a problem? Did they not want us to see how our mother was being treated? In the week that my mom was there, she would cry and we were heartbroken to see her like that. My mother is a very lively person and her (herself) recognized that she lacked the treatment that she deserved. The straw that broke the camel's back was when my mom was to be transported to a dialysis appointment. She was sent off without any clothes or diaper. She was sent off with a blanket over her and if it wasn't for the transporter to point that out, I would not have known that she was sent off naked and to make matters worse, it was on a cold day. I had to run to the nearest store to get her clothes and the dialysis center was kind enough to allow us a space so that we can dress her. Please DO NOT trust these people at the center. They will sell you a dream place and they are FAR from it. Their lack of compassion, respect and attention to their patients is unbelievable and inhumane. PLEASE PLEASE PLEASE... IF YOU LOVE YOUR FAMILY MEMBER / FRIEND, PLEASE PLEASE PLEASE DO NOT SEND THEM TO THIS CENTER. STAY CLEAR AWAY FROM THEM!

This was one of the worse centers my husband was ever in. The Staffing of nurses really were far out there. They seemed to worry about their personal life and some wedding plans that overcome the responsibility of caring for my husband. They seemed to be more into discussing their personal life and forgetting they had patients. my husband did not remain there after it was discovered that he had not been bathed him in a few days, When asked about it they had an excuse especially the RN that they did not have enough staff to bathed him, But then she would turn around and engage her friends in a personal conversation with no regards to the patients. I think you guys need to stop the personal chit chat and separate that group if not replace them. If there would have been a better experience I would not had to move my husband, He has just recently passed and I am going to call for a state investigation of this facility.

THE PLACE IS NOT WELL STAFFED THE CNAS STAND UP AGAINST THE WALLS WITH THE FEET ON THEM THE FLOORS ARE NEEDING REPAIRS AND THE ROOMS ARE TO SMALL NOT ENOUGH ROOM FOR VISTERS AND 2 RESIDENTS PER ROOM, SMELLS OF HUMAN FLUIDS LAUNDRY LOOSES ITEMS OR PLACES THEM IN THE WRONG PLACES, i WOULD NOT PUT ANY FAMILY MEMBER IN THIS PLACE EVER AND i SURELY WONT TELL ANY ONE OF THIS DUMP.....

My daddy, the late Lloyd Moore, lived in this facility from 1974-1996. He wanted to pass at Community Convalescence Center. Daddy always told me that if you can keep your love one at home, do so. If they have to go into an institution, he/she needs to be able to speak up, by asking for what you need or want or reporting mistreatment. I think Daddy liked living here and the services had to have met his needs. He was 88 when he passed. I sincerely thank and appreciate the staff for the services that they provides to my daddy for the 22 years that he was a resident at the Center.

This place is terrible. It smells of urine. they would rarely answer the phone when We called from 4 states away. Whent hey did answer the phone would not reach my step father room???
They had my step father on a rubber mattress with no sheet for day on end. As a result he developed bed sores.
He was of sound mind for his short stay. he was unable to use his hands so he asked the nurse to place his meds in his mouth for him. She routinley woul dno do so and stated " it was not her job"! We came down from the northeast to visit after having concerns abbout his welfare in this place. We ended up moving my stepfather immediately upon arrival !!
