My mother was admitted to that facilities.
Immediately they asked her to fill up review questioner, even she just came in. I complained about it, but they insisted. I had to put all the best answers.

They put my mother to share a room with some "hard" patient.
She needed the ventilator. They had three of them, and all of them were broken.

Employees were not skilled or interested to help anybody. I asked for some help, couple of them promised to come and nobody came.

My mother's oxygen level dropped really low, and after even a few days she passed away.
