Your survey does not cover a review of the facility. My brother was there for physical theropy,,, which was stopped in the middle because the theropist decided he was not giving his all. After several months it was started back up after we talked of a lawyer. They lost all his clothes several times (size 10X) and now refuse to pay for them after they told him upon release they would. And yes the place usually smelled.
