This therapist was appointed by the court in a family law matter. He asked for the judge to order a "waiver of liability" - I'm not even sure if that's legal, but I wouldn't want to go to a provider who didn't want to be accountable for his actions. Like you want to go to a surgeon, who made you sign a release stating that he wouldn't be responsible for any mistake or other harm that he might cause, to operate on you? I think not. There's plenty of other therapists out there who don't go around asking to be not held liable for negligence or malpractice, I'd go find one.
