They are only concerned about your copay and they do not want to bill the insurance company. They want you to pay upfront on certain things that the insurance will cover.

my problem is with the office , $25..0 for one form , when my husband went back to work he needed another back to work form, there's another $25.00 so a total of $50.00 for two forms

Rude staff, ungodly waiting room time, incorrect diagnosis, trying to charge for appliances I did not need and which were not warranted for the condition I was misdiagnosed for. The practice is a sham. And to top it off they overcharged my insurance carrier, had the audacity to overbill me for the difference in their charges versus what they agreed to accept. AND I had to go to another doctor for a correct diagnosis. They told me it was my foot when the issue was with my ANKLE. Close but not the same part. Had the doctor spent any time with me perhaps he might have realized it. A TOTAL WASTE OF TIME. DO NOT GO, save yourself from the incompetence

The visit corrected the problem. However, without notifying of an extra charge, slammed with a "tray" fee of $35.00 which is not covered by insurance ridiculous. Then harassed about the fee and threated by a $10.00 fee on an extortion of $35.00 Not going back.
